    COI (Certificate Of Insurance) Request Guidelines

    Thank you for sanctioning your race or training events with Triathlon Ontario!
    • Please note that this form is for requesting a COI for a sanctioned event.
    • You are welcome to request a COI by completing this form at any time - even before your event is sanctioned - however a COI will be issued to you only once sanctioning is complete.
    • It's a good idea to put in your COI request at the same time as your sanction application. This lets us get the COI from the insurer and iron out any problems while the sanction is being approved.
    • To receive a COI, your sanction application must be complete, paid if required, and approved by Triathlon Ontario.

    • All sanctioned events are insured as soon as they are sanctioned.
    • Not every event requires a COI. Typically, a COI will be required when a venue, municipality, or other organization will not provide access or services unless presented with proof of insurance.
    • A COI is not required for a sanctioned event to be insured, only to prove insurance is present. If no-one requires proof of insurance for your event to proceed, you don't need a COI.
    • If you do need a COI, be sure to request one well before your event, particularly if unusual coverage or increased liability is required.

  • Event Information

  • The type of event or activity for which you are requesting a certificate. Club workouts and clinics are considered club activities.
  • Name of the event or club activity for which you are requesting a COI.
  • The date of the event, the start date of a multi-day event, or the first date of a club activity.
    Date Format: YYYY dash MM dash DD
  • The end date of a multi-day event or club activity. Not required for one-day events.

  • Name of the location or venue at which the event takes place. Examples: Ontario Place, Guelph Lake Conservation Area, Baden Community Centre, Queen's University Pool, Leslie Rd, between Milborough Line & Victoria Rd, etc. For club activities which involve multiple locations, enter "Various Locations."
  • Street address of the location or venue the event takes place. For club activities which involve multiple locations, enter "Various Locations" and the municipality.

  • Contact Information

  • Name of the organization, club, or LOC (Local Organizing Committee) running the event.
  • Street address of the Organization or Club running the event.
  • Your name as the contact for this request.
  • Your current email address. We will use this address to contact you regarding this request.

  • Certificate Holder

  • If required, you can designate one (and only one) Certificate Holder per certificate. This provides no coverage, but some venues insist on being the Holder. If your Holder also requires coverage, be sure to list them as Additional Insured as well. There is no need to designate a certificate holder unless an organization is insisting on it.

    Notes on Entering Additional Insured

    In order to avoid delays, changes, and resubmissions, please adhere to the following when entering additional insured:
    • Enter only additional insured who legitimately require being named on a certificate. Examples of additional insured who often insist on being explicitly named are venues, municipalities, ministries, and police services.
    • The following are already covered and may not be entered as additional insured: race directors, staff, volunteers, sponsors, organizers, clubs, coaches, board members, and local organizing committees.
    • Especially in the case of government agencies and ministries, please ensure that the additional insured are entered exactly as they need to appear on the completed certificate. Some agencies will hold up approvals based on extremely small errors such as typos.
    • Be sure to enter street addresses in full including postal code. An incomplete address or missing postal code will cause a rejected certificate.
    • You can enter as many additional insured as you need to by clicking on the plus sign (+) on the right.

    Confirmation

    • On submission of this form, you'll receive a confirmation email with the information you've submitted.
    • Once your event sanctioning is complete and approved, a COI will be emailed to you.
    • If changes are required, use this form to submit an updated request. We cannot accept verbal or email changes.
